Payment in full is due at time of service unless prior financial arrangements have been made in advance.
We accept Visa, MasterCard, Cash & check.  We also accept low/no interest financial payments offered through Care Credit. Cash payments in full made the day treatment is started will be entitled to a 5 % discount.
We participate in Delta Premier Insurance Plan.  For all other insurance plans we ask for payment from you. We will gladly assist you by filing your insurance claims electronically.
Dental insurance payments are your responsibility as it is a contract between you and your insurer.
Due to the complexities of different coverage options within many different plans and companies, we cannot guarantee insurance reimbursement. We will always do our best to assist you in the filing of claims.
Missed appointments- Appointment times are reserved exclusively for you. 24 hour notice is appreciated if you are unable to keep your appointment.
Service charges- a 1.5 % per month interest (18% per year) will be charged on accounts with unpaid balances.
Returned checks- there will be a $ 35.00 charge for any returned checks.
